The Walgreens Health Info Card has the information your health professionals need. For just an initial enrollment fee of $9.99 you can get your own personal Walgreens Health Info Card.

  1. How to Enroll
    1. Visit Your Account on Walgreens.com and click on Access Your Health Information. Click on Get Program Details.
    2. The Get Program Details page explains the program. To begin the enrollment process, simply click Not a Member? Join Program.
    3. Provide all the required information and add any additional notes that you would like a health care professional or other authorized person to know. This includes your health history, immunization records, emergency contact information, physical statistics and additional notes (ex: "I use blood thinners regularly"). Please review all your information before you click Continue.
    4. Accept the Health Info Card program Terms of Use and your checkout process will begin.

  2. How to Update Your Information
    1. Visit Your Account on Walgreens.com
    2. Click on Access Your Health Information.
      This will take you to Your Health Info Card Account.
    3. On Your Health Info Card Account page, click on Your Walgreens Health Information. On this page you can begin to edit your enrollment information, including,
      • Personal Information
      • Health History Information
      • Your Immunization Record
      • Emergency Contact Information
      • Primary Care Physician
      • Your Physical Statistics and Notes

  3. How to Access and Print Your Report
    1. Visit Your Account on Walgreens.com.
    2. Click on Access Your Health Information.
      This will take you to Your Health Info Card Account.
    3. On Your Health Info Card Account page, click on View/Print Your Walgreens Health Info Card Report.

      This will take you to a printer-friendly version of your Health Info Card Report. Use your browser’s Print button to print a copy of your report.

  4. How Your Information is Kept Secure

    Walgreens is committed to securing your personal information. We use technology that is designed for use with secure Web servers. Prescription drug information resides in a database behind a firewall where it cannot be accessed without proper authorization. Secure Sockets Layer ("SSL") technology encrypts your personal information as well as your prescription health history as it is transmitted over the Internet.   5. Expiration of Membership and Renewal Fee

    Your Health Info Card expires in 12 months from the date of registration. The exact expiration date appears on the front of the card. At that time, you can renew your membership for only $5. Be sure to keep your card current with any new health information.
